Unofficial administrative transcripts

Administrative Request - Special Investigators

Please complete the below form to submit a request for an unofficial administrative transcript. Requests will be reviewed by the University Registrar's office and processed in the order that they are received.

Please ensure you have completed each step required to submit your request. Upon submission, you will receive an automated email confirmation. If you do not receive a confirmation, please review and resubmit your request.

Note: Upon verification of an academic record, you will receive an email notification indicating when the transcript will be available for pick up. If no record is found or if a transcript cannot be released, you will receive an email notification. 

Unofficial administrative transcripts will be available for pick up only Monday through Friday, 11:00 a.m. to 4:00 p.m. EST in Buckeye Link, located on the first floor in the Student Academic Services (SAS) Building, 281 West Lane Avenue, Columbus, OH 43210.

A signed authorization to release information from the student/alum is required at the time of pick up. We will retain the authorization form. Transcripts cannot be released without receipt of authorization.

  • Transcripts cannot be faxed or emailed.
  • The Registrar’s office is not authorized to access encrypted files for transcript requests.
  • Transcript requests cannot be accepted by telephone.

Alternatively, written requests for unofficial transcripts can be submitted in person at Buckeye Link. On agency letterhead, the request should include the student/alum’s name, date of birth, approximate dates of attendance, your name, title, agency, telephone number and email address. Please ask the Buckeye Link staff member to forward the request to the Registrar’s office, Attn: T. Cook. Transcript requests will not  be processed immediately.

The University Registrar's office is located at 540 Student Academic Services Building, 281 W Lane Ave, Columbus OH 43210. View our location on the Ohio State campus map. Parking is available in the Lane Ave Garage.
