Circulating Forms Archive

The Circulating Forms Archive provides a record of changes to existing courses, new course proposals, and course withdrawals. Individuals seeking additional details about items listed in the archive should contact the University Registrar’s Office, the Office of Academic Affairs, or the academic unit responsible for the request.

 

Notifications of newly archived circulating forms are distributed through a campus listserv, and requests to subscribe or unsubscribe should be directed to the University Registrar’s Office.

 

Each circulating form remains open for review for two weeks after publication; if no objections are submitted to both the Office of Academic Affairs and the proposing academic unit during this period, the request is considered approved.

 

Circulating forms are archived during the first week of each month and include materials approved by Academic Affairs and received by the University Registrar’s Office in the previous month. Publication years for circulating forms run from March through February.

 

The materials in this Circulating Forms archive are intended solely for research, reference, and recordkeeping purposes and may not be used for current instruction, programs, services, or public communication.
